Referral and Application Process
Welcome to the TouchPoint Admissions
Department! Our Admissions Department is the
central point of intake for all TouchPoint
services. Each person must complete the admissions process
prior to being accepted for any TouchPoint service.
Admissions Specialists are available to receive inquires and
referrals, and to guide you through the admissions process.
Referrals for admission may be made at any time, and are often made
by pediatricians, neurologists, case managers, social service or
education professionals, or by parent or guardian.
Tours of our programs may be arranged on a case-by-case basis, and
coordinated to meet the needs and preferences of the individuals
whom are served by TouchPoint programs.
To access TouchPoint services, please complete the following
steps:
STEP 1: Complete a TouchPoint
Application.
Download the TouchPoint Application, or contact the Admissions
Department to have one mailed to you. After completing
the TouchPoint Application, please refer to the bottom of the third
page of the Application and provide all available information as is
appropriate for the applicant. It is especially important to
provide a copy of the applicant's Social Security Card as well as a
copy of a document that verifies the applicant's diagnosis (this
can be a document from the applicant's diagnosing specialist).
Please note: An applicant does not need to have a
diagnosis to access our ADOS assessment services or on-site ADAPT
services. All other services require a diagnosis.
*In order to be considered eligible to receive selected grant
funding opportunities available for certain consultation services
provided through TouchPoint, this information is mandatory, but is
not a guarantee for access to grant funding.
STEP 2: Complete and Sign the
"Acknowledgement of Receipt of Notice of Privacy
Practices" (page 1) of the HIPAA Policy information.
STEP 3: Mail all completed and signed documents
to:

Upon receipt of the application (and attachments), an Admissions
Specialist will contact you to verify its receipt and to discuss
the next step(s) toward accessing TouchPoint services.
